Mail Merge Query Submission Forms for CCHelper sites

Michael Hebert's Using Mailmerge For Queries tells you how to setup a query submission form with various options, including special instructions for CCHelper. Michael tells you how to have the query sent to you via e-mail or posted to a file. His instructions tell you how to use mailmerge on a server besides Rootsweb, if the server allows it.
I use the QueryExpress system to get my CCHelper queries. This system lets you choose whether the queries are mailed to you, posted to a temporary Web page, or posted to a mailing list. You can choose any combination of these options.
The queries sent to you or posted to the temporary Web page can be in CCHelper format, ready to be imported into CCHelper without any massaging. I have my queries posted to a temporary Web page and then collect them with one copy-and-paste command when I am ready to index them.
